The calculator separates what is recurring (losses avoided and financial gain) from what happens once (cash released), following Forrester's Total Economic Impact methodology and HFMA MAP Keys.
| Lever | Conservative | Base | Aggressive |
|---|---|---|---|
| Losses avoided (on production) | 2% | 3% | 3% |
| Production billed in the month (target) | 85% | 95% | 95% |
| Target billing lag | 20 days | 15 days | 10 days |
| Initial denial reduction | 45% | 60% | 70% |
| Faster claim building and review | 50% | 70% | 75% |
| Reduction in claims under appeal | 45% | 60% | 70% |
Calculation rules
- Losses avoided: monthly production × 12 × scenario percentage. Claims reflect 100% of the care delivered.
- Average collection period = current period − billing lag reduction − (same-month target − % billed today) × 30 days − initial denials × reduction × 60 days of appeal. Floor of 30 days.
- Cash released = annual production ÷ 365 × days reduced in collection. Financial gain = cash released × cost of capital.
- Initial denials avoided is money that arrives on the first payment, not new revenue. It is not included in recurring impact.
- Team: if the number on denial appeals is not provided, 16% of revenue cycle staff is used. Each person has 160 productive hours per month.
- Hours returned = production hours × claim-building gain + appeal hours × reduction in claims under appeal.
- Counterpoint without Accrua: additional billing specialists = production staff × (same-month target ÷ % billed today − 1); with no team provided, 240 claims per specialist/month is used. Additional auditors = claims × 20 min of pre-audit ÷ 160 h. Monthly cost with payroll taxes: R$ 6.5K per billing specialist and R$ 13K per auditor.
- Cash released and recurring impact are never added together.
